For your first visit, please arrive 10 minutes before your scheduled appointment time. Also, fill out the health and personal information sheets before your arrival and bring them with you along with your insurance card and photo identification the day of your visit. The cost for your cosmetic consultation will be $85.00. For insurance consultations, you will pay your standard co-pay.

Park in the GREEN SECTION LEVELS of the visitor parking deck. You must take a ticket to enter the parking and bring this inside with you.  The Green Level crosswalk on level three will take you to the Ambulatory Care Center (ACC) building, where you will find The Duquette Center on the fifth floor in Suite 511. The fee when leaving the parking deck is $3.00.  You may pay at machine located by the parking elevator or on your way out of the parking deck.   We are not able to validate parking passes at this time. Valet parking is also available in front of the Women's Medical Plaza for $7.00.
